How do you prepare marriage documents and get the health report?
A step-by-step route to assembling the file for the Marriage Registry and obtaining the premarital health report.

Identify the registry office you will apply to
The ceremony can be performed at the Marriage Registry of the municipality where either partner is registered as a resident. Open the "Evlendirme İşlemleri" or "Nikah İşlemleri" page on the municipality website; the document list and application method are there. In large cities the list can differ slightly between district municipalities.
Book an appointment
Many municipalities only accept applications by appointment. On the e-Devlet portal, type your municipality's name into the search box and look for the "Evlendirme Randevusu" service. If it is not listed, use the "Online İşlemler" or "e-Belediye" section of the municipality's own site, or call the Beyaz Masa desk.
Fill in the marriage declaration form
You can collect the form in person at the registry or download and print it from the "Formlar" or "Belgeler" section of the municipality site. Each party signs their own section. Enter identity details exactly as they are spelled on the ID card, since a single letter mismatch will send the file back.
Book a family doctor appointment for the health report
The premarital health report is issued by the Family Health Centre you are registered with. Create an appointment through the MHRS app or mhrs.gov.tr using "Randevu Al" and choosing "Aile Hekimliği", or call ALO 182 if you prefer the phone. Tell the doctor you are there for a marriage report.
Complete the tests and have the report signed
The doctor orders the tests required by regulation and sends you to the blood collection unit. Once results are in, go back to the doctor to get the report signed and stamped. Some municipalities issue the report at their own partner health unit, so ask the registry first and you may save yourself a second trip.
Gather documents for special circumstances
If a previous marriage ended in divorce, you need the divorce annotation on your civil registry extract; if a spouse has died, the death record. A foreign partner obtains a certificate of capacity to marry from their country's mission and has all documents sworn-translated into Turkish.
Hand in the file and set the wedding date
Both parties go to the Marriage Registry together with their ID cards and submit the declaration, photos and health report. After the officer checks the documents, you choose the hall and date and pay the ceremony and booklet fees. Keep the marriage permit document you receive until the ceremony.
Arrive on the day with your witnesses
Both parties must carry their ID cards, and two adult witnesses with sound judgement must attend with their own ID cards. The marriage booklet is handed to you at the end of the ceremony, and the officer reports the record to the civil registry directorate.

The most common mistake is submitting the health report or civil registry documents after their validity has expired, which means starting the whole process again. Document lists also vary between municipalities, so confirm with your own Marriage Registry which document is wanted in which form: original, barcoded printout, or translated.

Some municipalities accept only reports from a Family Health Centre or a state hospital. Ask the registry office before going to a private institution.
